Job Description:
Reporting to the Manager, Chronic Kidney Disease, Home Dialysis Program, the CKD Nurse Clinician facilitates the overall daily functioning of the Chronic Kidney Disease Program, ensuring a high quality of patient care services. The incumbent collaborates with the interdisciplinary team members and liaises with health professionals and agencies within and external to the Saskatchewan Health Authority to facilitate chronic kidney disease education and management.
Travel Required: Yes; Travel may be required to support Community Screening Program work.

Experience
- Minimum of three (3) years of full-time clinical Nephrology experience in the past five (5) years. Minimum of one (1) year experience in program/resource development and evaluation. Recent experience in charge nurse/leadership role is required. Nursing experience in Chronic Disease Management is desired. Experience in education or a teaching role is desired.
Education
- Bachelor of Science in Nursing (BScN)
- Nephrology certification is desired.
Licenses
- Basic Life Support Certificate (BLS)
Other Education and Training
- Nursing diploma is acceptable
Knowledge and Abilities
- Demonstrates effective capacity for coordination, planning, development, quality improvement and evaluation of the CKD program, thorough knowledge of chronic kidney disease and chronic care models.
- Strong leadership skills and organizational abilities are essential.
- Self-directed and requires minimal supervision.
- Excellent verbal and written communication skills.
- Demonstrates highly developed clinical assessment skills.
- Knowledge of hospital and community resources to ensure continuity of care.
- Demonstrates ability for critical thinking, problem solving and decision making.
- Demonstrates ability to work effectively and cooperatively in an integrated interdisciplinary team.
- Coordinates and facilitates development and functioning of the interdisciplinary team.
- Demonstrates ability to accept and implement change.
- Demonstrates ability to work as a team member establishing and maintaining effective interpersonal/consultative relationships.
- Demonstrates knowledge and understanding of current collective agreements and the ability to apply this knowledge in the development of rosters and scheduling, collaboratively with the Manager.
- Computer skills and working knowledge of current software programs.
