POSITION SUMMARY: As a member of the primary care team, the Nurse Practitioner (NP) is accountable to the Chief Executive Officer and Medical Director for meeting the Long Term Care Home’s policies, procedures and protocols for medical services.
Working in collaboration with the Medical Director and/or Attending Physicians, the Nurse Practitioner will provide a range of health services to residents, and deliver preventative and continuous care towards the management of the residents’ health.

QUALIFICATIONS:
- College of Nurses of Ontario registration in the Extended Class.
- Baccalaureate in Nursing (BScN), Masters Degree in Nursing (MScN or MN) an asset.
- Knowledge of Nurse Practitioners’ Standards of Practice as set out by the College of Nurses of Ontario.
- Membership with Nurse Practitioners’ Association of Ontario (NPAO).
- Member of Registered Nurses’ Association of Ontario (RNAO) with professional liability insurance.
- Experience in long term care an asset.
- Experience in palliative care/end of life care, dementia care an asset.
- Knowledge of RNAO Best Practice guidelines and MOH/MLTC Regulations.
- Knowledge and understanding of the role of the Nurse Practitioner within an interdisciplinary team and how to optimize collaborative relationships.
- Demonstrated competency in advanced assessments, diagnosis and prescribing pharmaceuticals and non-pharmacological interventions within the legislated scope of practice.
- Clear understanding of Resident Centred Care.
Responsibilities
- 70% of the NP’s time must be engaged in direct care to residents; 30% to be engaged in research, education and leadership for the Registered Nurses and/or Registered Practical Nurses.
- Provide primary care within the scope of practice of the NP as outlined in the College of Nurses of Ontario Standards of Practice for Registered Nurses in Extended Class. The provision of primary care includes assessments, diagnosis, counselling, screening, referral, education, treatment and follow-up.
- Collaborate effectively with physicians to facilitate appropriate provision of primary care outside of scope of practice.
- Liaise with physicians and community agencies as required.
-Maintain complete, accurate and timely records of patient visits using the electronic medical record (EMAR) and electronic treatment record (ETAR).
- Communicate findings and diagnoses, discuss health outcomes to residents, families and care team.
- Work effectively within an interdisciplinary team to improve resident care, and improve the team’s ability to provide a broader range of services (eg- wound care management).
- Evaluate and analyze residents’ response to therapeutic interventions.
- Participates in internal and external committees (eg- Professional Advisory Committee).
- Provide leadership and involve the inter-disciplinary team in quality improvement initiatives.
- Provide leadership in developing and implementing strategies to optimize the integration of illness and injury prevention, health promotion, health maintenance, rehabilitation and restorative care activities.
- Provide formal and informal teaching and coaching in the management of clinical care to interdisciplinary team members, serving as a resource person, educator and role model.
- Provide on-call services in limited physician absences.
- Assist in maintaining a physical and psycho-social environment which meets the needs of the residents.
- Deal tactfully and courteously with residents, residents’ families, visitors and staff.
- Interacts consistently with residents in a positive manner.
- In the performance of all duties, demonstrate respect for the privacy, dignity, and individuality of the resident; adhere to home policy on confidentiality.
- Perform all duties and demonstrate conduct in accordance with the policies and procedures of West Perth Village.
