East Kootenay Primary Care Network has a relief, full-time opportunity for a Bachelor’s level Social Worker supporting Invermere’s Health Centre/Primary Care Network Team!

About Primary Care | A Primary Care Network (PCN) is a model of care that that supports the delivery of team‐based primary health care services in a local area. In a PCN, family physicians, other primary care providers, and allied health care providers in a geographic area work together to meet the health needs of their local community. East Kootenay PCN clinical teams are patient‐focused, coordinated teams and include physicians, nurse practitioners, nurses, allied health providers (social workers, occupational therapists, physiotherapists, mental wellness clinicians, dietitians, pharmacist, respiratory therapists) and Indigenous health coordinators actively working together to address the primary care needs of East Kootenay residents.
About the job | PCN Social Workers can help individuals manage a variety of emotional and social concerns that impact their health and wellbeing. They can assist with housing and food security, mental health concerns, identity needs, and crisis and safety needs. They support connection of individuals with appropriate community services, assist with access to benefit services, and coordinate with individuals’ healthcare teams. This position’s shift times are from Monday to Friday 08:30 to 16:30 hours. This position is to provide coverage until return of incumbent (date unknown).

Qualfications
• Bachelor’s Degree in Social Work from an accredited university.
• Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
• Current full registration with the British Columbia College of Social Workers.
• Current valid driver’s license.
