Position Summary
Joining us in Creston, B.C., you’ll have a pivotal role providing social work services to in-patients attached to the Creston Valley Hospital (CVH). You will provide comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, you will provide direct psychosocial care, counseling, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self-management.

Key Responsibilities:

• Provide urgent responses, psychiatric assessments, and treatment planning for individuals referred to psychiatric services.
• Offer supportive treatment for patients in both short-term and long-term care, helping them navigate complex psychological needs.
• Work collaboratively with a multidisciplinary team to ensure comprehensive care for patients.
• Support the emotional, mental, and social well-being of patients while assisting them in developing coping strategies and accessing necessary resources.

This is a Casual opportunity. Shift type(s) for this position are days as needed and shifts of work are from 08:00 to 16:00 hours. Casual means there are no guaranteed hours and work requirements can vary from Zero to 37.5 hours per week. Qualfications
• Bachelor’s Degree in Social Work from an accredited university.
• Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
• Current full registration with the British Columbia College of Social Workers.
• Current valid driver’s license.
