Advanced Practice Nurse - Trauma Performance Improvement (PI) Coordinator and Neurosurgery Education Outreach Network Outreach Nurse

In collaboration with other Tory Trauma Program and unit based leaders the APN will lead and participate in quality improvement initiatives, education, research, and program planning and implementation.


Trauma PI specific responsibilities

  • Identify areas for improvement in trauma patient populations related to quality of care.
  • Propose and implement potential solutions and course of actions to address identified areas for improvement.
  • Complete primary event reviews, event capture and validation, review triggers in real-time.
  • Prepare materials for secondary and tertiary review, relevant accreditation, and the PI Plan.
  • Attend morning rounds (trauma surgery, unit-based) as needed.
  • Represent Sunnybrook PI activities internally and externally in local, provincial, and international meetings.

NEON specific responsibilities

  • Collaborate with internal and external inter-professional team members to improve the quality and continuity of care for patients repatriating to non-neurosurgical centres.
  • Act as an ongoing support/ liaison for teams at non-specialized centres caring for repatriated neurosurgical and trauma patients, including discharge planning to ensure access to specialized community programs and services.
  • Travel to non-neurosurgical centers as needed to facilitate care transitions, provide education, and build capacity.
  • Provide education to patients and families recovering from neurosurgical diagnoses and traumatic injuries and act as an ongoing support/ liaison following repatriation, including system navigation and access to specialized services and programs in the community.
  • Lead and participate in internal and external committees, working groups, and research initiatives at the regional and system level, including collaborating with NEON Outreach Nurses at UHN and Unity Health.

Qualifications/Skills

  • Current College of Nurses of Ontario Certificate of Competence;
  • Trauma and neurosurgical experience required;
  • Master’s degree (completed/in progress) ;
  • Critical care nursing experience preferred;
  • Knowledge related to process improvement/rapid cycle improvement, change management, human factors, patient safety, risk management, and evaluation methods preferred;
  • Program planning, implementation, and evaluation skills;
  • Demonstrated professional leadership and collaboration skills;
  • Recent demonstration of leadership in quality and research initiatives;
  • Strong track record of quality and research presentations and dissemination of knowledge;
  • Excellent interpersonal and communication skills;
  • Demonstrated satisfactory job performance and attendance.
