Director, Community Integration and Outpatient Transformation

 

 

Position Summary:

 

As a member of the Community Integration and Outpatient Care portfolio, the Director, Community Integration and Outpatient Transformation will play a critical role in the deliver of portfolio priorites – to enhance primary care; foster strategic home care partnerships; and advancing outpatent care transformation.

 

Reporting to the Vice President, Community Integration and Outpatient Care, the Director will work in collaboration with the newly created Medcial Director, Community Integration and Outpatient Transformation, to foster collaboration across internal programs, maintaining Sunnybrook’s position as a leading provider of outpatient care services. The role will support the planning, analysis, design and evaluation of new or enhanced structures and processes within outpatient care settings, in alignment with corporate strategic direction #2 (Integrated and Sustainable Models of Care) and the Hospital’s Outpatient Care Strategy. In partnership with Program Operations Directors, Department Chiefs and Physician Leads, the successful candidate will support the project implementation and performance monitoring for all aspects of the Outpatient Care Strategy.

 

The role will also have operational oversight of the Reactivation Care Centre (RCC) beds, stationed at the Humber Church site, promoting hospital flow and transitions. It will also serve as the operational point of contact for partners, in the community support services led model of Pine Villa, Sunnybrook’s community transitional beds.  The Director will implement new transitional and community care programs to support patient flow from hospital and the provision of care in community-based settings.

 

Program Planning and Design

  • Lead the implementation of Sunnybrook’s Outpatient Care Strategy:
    • Implement a robust governance structure that encourages accountability, transparency and collaboration to ensure outpatient care effectiveness across all programs.
    • Analyze current outpatient care processes and workplans to idenfify commonalities and areas for further development across programs.
    • Spearhead relevant Outpatient Care Strategy working groups to support the work and standardize outpatient care delivery.  
    • Work with departments and programs to implement pollicies and procedures/initiatives to optimize patient experience and satisfaction in outpatient settings.
    • Collaborate with the program leadership, Decision Support and Finance in the development of performance measures and outpatient care dashboards that align to a clear clinical and operational performance management framework.
    • Develop strategies to ensure optimal Patient, Staff and Provider engagement across all outpatient care settings.
    • Work in close partnership with Digital Health team to ensure a successful Hospital Information System (HIS) implementation in outpatient care settings.
    • Collaborate with Programs and Corporate Planning and Development to ensure a state of preparedness to operationalize the new outpatient clinic space, incorporating outpatient care principles and end-state goals.
    • Work with program leadership to establish goals and objectives to optimize outpatient care and services provided for target populations and identify strategies for growth.
  • Partner with program leadership to identify, implement, maintain and evaluate improvement of quality and safety initiatives in the outpatient setting in alignment with Sunnybrook’s Quality Improvement Plans and Hospital Accreditation.
  • Collaborate with professional practice team to implement a standardized approach to utilization and deployment of professional practice roles, and deployment of models of care assessments in outpatient care settings.
  • Champion the involvement of patient and family advisors in co-designing and planning of outpatient care services.

 

Operational Leadership

 

  • Provide director-level clinical operational oversight of the RCC beds (60 beds, 2 units). This includes the support of the RCC Patient Care Manager and teams, and serving as the director level Sunnybrook liaison with Humber River Church site contacts.
  • Serve as the operational point of contact for partners, in the community support services led model of Pine Villa, the 68 community transitional beds. This includes the Sunnybrook clinical teams at Pine Villa.
  • In collaboration with program leadership and the Patient Flow team, work to support the transition of Sunnybrook patients to the appropriate post-acute transitional care or community setting, contributing to operational patient flow discussions.
  • Promote principles of a healthy work environment, shared governance, staff empowerment and distributed leadership to optimize the use of organizational roles to full scope and encourage a culture of engagement, accountability and collaboration
  • Represent Sunnybrook with key stakeholder groups, committees and initiatives to advance vision and mission
  • Promote a positive and progressive image for Sunnybrook with both internal and external stakeholders

 

Education and Experience

 

  • Master’s degree required (preferably in nursing or health related discipline and/or business).
  • Ten years of recent and related progressive senior level experience in health care
  • Extensive knowledge of the healthcare sector with experience in outpatient care setting
  • Extensive knowledge of financial and performance management strategies in the hospital sector
  • Working knowledge and application of relevant legislation including the Excellent Care for All Act and FIPA/PHIPA
  • Demonstrated knowledge and experience with performance measurement, data reporting and analysis
  • Demonstrated experience in the development and implementation of effective strategies to maximize interprofessional collaboration
  • Proven project and change management experience; recognized as an influential change agent with the ability to achieve ongoing process improvement
  • Superior interpersonal, written and verbal communication skills, including report writing, proposal development and presentation skills; recognized as an open, honest, and transparent communicator.
  • Proven demonstration of workplace excellence shown by commitment to strong job performance and attendance
