Description
Sunnybrook is the Regional Stroke Centre (RSC) for the North & East GTA Stroke Network. The Regional Stroke Best Practice Team is an interprofessional team that functions in both a regional leadership and clinical capacity. The team provides stroke leadership and education for allied health professionals and supports the development and implementation of best practice stroke care related to the assessment, rehabilitation, and recovery needs of persons with stroke across the continuum. Their role facilitates access to stroke expertise, case management, and consultation services.
Summary of Duties:
Reporting to the Director of the Regional Stroke Program, the Occupational Therapist (OT) on the Regional Stroke Best Practice team will work collaboratively to support development of region-wide stroke initiatives and implementation of these initiatives at partner sites. As a member of the Best Practice team, the OT provides consultative services to patients followed by the Secondary Stroke Prevention Clinic and will also support emerging priorities within the Regional Stroke Centre. The OT will be a champion in promoting excellent patient care while also demonstrating exceptional leadership, communication, problem-solving and conflict resolution.
Responsibilities Include:
- Work collaboratively with the other members of the Regional Stroke Best Practice Team to develop and lead regional initiatives across all partner sites of the North & East GTA Stroke Network
- Act as a stroke best practice champion and provide linkages between clinical care and the regional stroke network
- Provide consultative occupational therapy services to the Secondary Stroke Prevention Clinic
- Identify the need for supplementary assessments or referrals
- Raise the awareness of community services to promote client independence, community reengagement, and self-management
- Act as a resource to the interprofessional team, patients and families regarding functional and cognitive changes after stroke
- Support the implementation and sustainability of the Toronto Stroke Networks’ (TSN) initiatives
- Actively participate in Regional Stroke Program and Sunnybrook meetings, committees and planning as required
- Actively participate in and/or lead TSNs’ committees, meetings and initiatives
- Maintain and upgrade skills and knowledge through continuing education and seek opportunities forknowledge dissemination
- Actively participate in interprofessional and clinical education activities
- Practice in accordance with College of Occupational Therapists of Ontario (COTO)
Qualifications/Skills
- Registration with the COTO required with a minimum two (2) years of experience
- Membership in CAOT and/or OSOT required
- Previous experience working with neurovascular patients required
- Cognitive assessment experience required, cognitive intervention experience preferred
- Current credentialing in AlphaFIM® Instrument preferred
- Experience with stroke best practices and the stroke system in Toronto
- Demonstrated experience with change leadership and promotion of evidence based stroke care
- Experience and commitment to quality improvement initiatives
- Experience providing patient/family education is essential
- Demonstrated knowledge of organizational priorities and OT scope of practice priorities
- Superior time management, organization, and prioritization skills in addressing and managing multiple tasks in a fast-paced environment
- Strong written and oral communication, critical thinking, care coordination, and time management skills
- Established knowledge of relevant health care system processes and community resources required
- Demonstrated commitment to student education
- Demonstrated commitment to Person-Centred Care
- Ability to work collaboratively as members of intra- and interprofessional teams
- Demonstrated commitment to maintain and upgrade skills and knowledge through continuing Education and professional development
- Excellent professional judgment, conflict resolution and interpersonal communication skills with patients, families, team members, students and community partners.
- Experience in making referrals to community partners an asset
