Job Description:
Primary Responsibilities:
Provide consultation for patient’s nutritional goals, collects, and interpret patient health data for assessment, complete ongoing monitoring and reassessment in collaboration with patients, caregivers, and the health care team
Through patient-centered approaches, using motivational interviewing, the RD will work to establish priorities, strategies, and goals that allow the patient to achieve optimal outcomes
Develop, implement, and deliver patient education focusing on self-management strategies to improve dietary habits
As an interprofessional team member, the RD fosters a culture of safety by minimizing patient harm, promoting the well-being of patients and health care providers, reducing the likelihood of adverse events, and communicating safety concerns
Complete administrative responsibilities, participate in assigned projects and working groups, conduct practice-based research and perform other functions consistent with the job classification to ensure excellence in the provision of care, if required
Qualifications
Completion of a Bachelor degree in a 4-year Dietitians of Canada (DC) accredited university food/nutrition program degree/discipline, or recognized equivalent
Successful completion of an accredited practical dietetic training program accredited by the Partnership for Dietetic Education and Practice in Canada, or equivalent
Registration with the College of Dietitians of Ontario
Valid driver's licence
Membership with Dietitians of Canada, preferred
Minimum of 2 years' experience working as a Registered Dietitian preferred
Experience in a hospital environment and in related specialties, preferred
Experience in virtual care, preferred
Experience using Cardiological clinical information system, preferred
Experience using EPIC clinical information system, preferred
Ability to speak a second language, preferred
Complies with the Code of Ethics and Rules of Conduct according to College of Dietitians of Ontario; recognizes and acts within the scope and limitations of the Dietitian's role
Ability to exercise sound clinical judgment and to function in an independent, self-directed manner
Ability to learn and perform applicable advanced nutrition assessment
