Job Description:

Primary Responsibilities:

  • Provide consultation for patient’s nutritional goals, collects, and interpret patient health data for assessment, complete ongoing monitoring and reassessment in collaboration with patients, caregivers, and the health care team

  • Through patient-centered approaches, using motivational interviewing, the RD will work to establish priorities, strategies, and goals that allow the patient to achieve optimal outcomes

  • Develop, implement, and deliver patient education focusing on self-management strategies to improve dietary habits

  • As an interprofessional team member, the RD fosters a culture of safety by minimizing patient harm, promoting the well-being of patients and health care providers, reducing the likelihood of adverse events, and communicating safety concerns

  • Complete administrative responsibilities, participate in assigned projects and working groups, conduct practice-based research and perform other functions consistent with the job classification to ensure excellence in the provision of care, if required

Qualifications

  • Completion of a Bachelor degree in a 4-year Dietitians of Canada (DC) accredited university food/nutrition program degree/discipline, or recognized equivalent

  • Successful completion of an accredited practical dietetic training program accredited by the Partnership for Dietetic Education and Practice in Canada, or equivalent

  • Registration with the College of Dietitians of Ontario

  • Valid driver's licence

  • Membership with Dietitians of Canada, preferred

  • Minimum of 2 years' experience working as a Registered Dietitian preferred

  • Experience in a hospital environment and in related specialties, preferred

  • Experience in virtual care, preferred

  • Experience using Cardiological clinical information system, preferred

  • Experience using EPIC clinical information system, preferred

  • Ability to speak a second language, preferred

  • Complies with the Code of Ethics and Rules of Conduct according to College of Dietitians of Ontario; recognizes and acts within the scope and limitations of the Dietitian's role

  • Ability to exercise sound clinical judgment and to function in an independent, self-directed manner

  • Ability to learn and perform applicable advanced nutrition assessment
