Critical to the functioning of any hospital is the need to manage patient flow - Patients in versus Patients out. Flow focusses on Orchestrating Patient in the Right Place, at the Right Time, for the Right Amount of Time - this applies to discharging patients out of UHN, or transitioning patients within UHN to a more appropriate care level. UHN follows a Home First Philosophy.

The manager discharge and transitions will have operational accountability for supporting development / standardization / education / implementation and improvement of patient discharge / ALC processes across UHN Acute sites. Leveraging the capabilities of epic (EMR), for every discharge the process ensures patients are well informed, discharges are planned and LOS is minimized. Manager will be accountable to collaborate with and provide coaching across all UHN programs and sites, physicians, clinical directors, and unit managers and leadership of supporting services to ensure discharge processes are standardized, well socialized and simplified so that all parties can follow. Planned patient discharges and transitions benefit both the patient and UHN as a whole.

To support patient centred discharges and transition, Patient Flow Manager Discharge and Transitions will work closely with Ontario Health at Home and our Community Service providers to enable seamless transitions. Building relationships with our Community Partners and developing structures to engage them on a regular basis in transitioning patients back to the community will be a key to success. Developing metrics with all teams to evaluate success will be critical to long-term success.

A successful candidate will be a dynamic, process thinker, able to envision the patients journey from admission to discharge, Candidate will work across sites, with many stakeholders to reinforce a Home First Philosophy and all relevant UHN policies.
