Reporting to the Patient Care Manager - Diabetes Education Program & Chronic Disease Wellness Centre, the Clinical Utilization Coordinator role is viewed as the primary coordinator and anchor of ambulatory services that enables and role models teamwork and communication, autonomous clinical practice, patient safety standards and a healthy workplace.
What must you have?
- Current Registration with the College of Nurses of Ontario as a Registered Nurse.
- Minimum 2 years’ experience in a leadership role.
- Leadership experience in facilitation, decision making, conflict resolution and clinical abilities.

- Facilitates the effective delivery of competent, compassionate patient centered care by monitoring and evaluating patient care processes and outcomes.
- Coordinates, facilitates, participates and directs projects with follow-through and appropriate follow-up.
- In collaboration with the Patient Care Manager, assists with coordination of clinic staff schedules and daily staff assignments to ensure appropriate resource utilization and skill mix.
- Assesses ongoing staffing needs according to workload and staff competency; re- assigns and adjusts staffing based upon patient care needs.
- Understands and communicates potential clinical equipment or supply issues to appropriate team members (Materials, Biomed, Sterile Processing, etc.).
- Informs Clinical Educator of further learning needs and provides update as to new hires progress.
- Provides ongoing feedback and coaching to staff.
- Collaborates with the Patient Care Manager for issue resolutions and projects to improve workflow processes throughout the clinics utilizing a proactive approach.
- Collaborates with different departments (pharmacy, radiology, lab, registration, finance) to resolve issues and improve workflow processes throughout the clinics.
- Collaborates with the Patient Care Manager in the interview process for new applicants.
- Promotes an atmosphere of open communication that facilitates staff input into decision-making, resolution of conflict, and collegiality among all health team members.
- Creates an environment that supports and advances professional practice, teamwork and communication through mentoring and role modeling application of "expert" clinical knowledge.
- Liaises with physicians and members of inter-professional team to ensure excellence in patient care.
- Facilitates and continuously supports quality and process improvement initiatives and risk management programs at the unit level including taking action, with Patient Care Manager support, to ensure patient and team safety by completing initial investigation of Risk incidents.
- Role models professional and organizational core competencies.
- Ability to communicate and collaborate with all members of the inter-professional team.

- Ability to support the clinical team in a professional manner.

- Ability to problem solve clinical issues with the support of the Patient Care Manager.

- Solid computer skills; experience with relevant healthcare applications.
- Communicates in an effective, professional manner
- Proven attendance record.

Preferably, your profile also includes:
- Baccalaureate Degree in Nursing
- Master’s Degree in Nursing
- Minimum 2-5 years years recent and relevant experience in cardiac and/ ambulatory nursing
Previous project management experience and participation in process improvement and quality improvement events
