Description
Summary of Duties
To provide a full range of social work services to Neurovascular/Stroke patients and their families. Services include psychosocial assessments, counselling, discharge planning, linkage to community resources and participating as a key member of the patient care team.
Qualifications/Skills
• Master’s Degree in Social Work
• Member in good standing of the Ontario College of SW and SSW
• Minimum 2 years of experience working as a MSW in a Hospital preferred
• Commitment to Person Centred Care Best Practices
• Excellent communication, organization skills and time management skills
• Strong teambuilding skills and conflict resolution skills
• Excellent assessment, interviewing and problem solving skills
• Knowledge of the health care system and community resources
• Superior ability to manage multiple tasks and set priorities in a fast-paced environment
• Knowledge of and past clinical experience in Inter Professional Collaboration and Inter Professional Education
• Commitment to student education
• Interest in exploring Academic Practice opportunities that address Patient and Family Care across the continuum of Health Care
