This position is for our Extendicare Oakview Place Home, located in Winnipeg, Manitoba

What you’ll be doing
  • Provides a complete resident admission process including tours, admission, discharge, and occupancy tracking.
  • Acts as liaison as appropriate, with families. Provide counselling or group sessions for resident and families as identified.
  • Maintains established department plans, policies, and procedures to meet regulatory and Extendicare standards, including quality assurance.
  • Manages the quality care programs including Guardian Angel, Health Tracking, Behaviour Management, Ethics, and Palliative Care.
  • Participates in providing in-service programs for residents and families as needed.
  • Maintains all required records, reports, statistics, etc. for both residents and department in accordance with policies, procedures, and legislative requirements.
  • Acts in an advocacy role as appropriate.
  • Satisfy Health Record as they pertain to the department (MDS experience an asset)
  • Liaises with government officials, Extendicare consultants, and other professionals in relation to resident care needs and department activities.


What you bring
  • Degree in Social Work from an accredited University preferred.
  • Previous experience in long-term care setting.
  • Knowledge of applicable legislation and regulations.
  • Competency in conducting individual counselling and group sessions.
  • Computer literate
