Job Description: The overall function of the assessor/coordinator (client care coordinator) position is to provide case management services which includes assessment, planning, monitoring and evaluation. The client/patient access services is primarily responsible for a single entry access and ongoing coordinator for a variety of community based services within SHA(Saskatoon).

Education
- Baccalaureate of Social Work degree
Other Education and Training
- Bachelor of Occupational Therapy is acceptable
- Bachelor of Nursing is acceptable
- Bachelor of Nutrition is acceptable
- Bachelor of Physical Therapy is acceptable
Knowledge and Abilities
- Knowledge in the SHA, community resources and applicable programming
Other Information
- Previous Client Assessment Experience is an Asset
- Travel required
- Preference to HSAS members as per the SAHO/HSAS Collective Agreement
