Job Description: This unique Social Work position works across two Saskatoon Health Region program areas (MHAS and Positive Living Program) with a primary focus being social case management support for persons infected with Human Immunodeficiency Virus (HIV) and or Hepatitis C within a hospital, outpatient and community setting. The incumbent will work as part of the Positive Living Program (PLP) team linking out-patient program clients to community supports, acting as a liaison where support is required to transition clients from acute care sites to community supports.The ultimate goal of the Social Worker is to help clients understand their diagnosis in the context of a chronic disease, with a focus on the prevention of HIV and HCV transmission through harm reduction strategies, supporting the client to stabilize their addiction issues and participate in collaborative planning with their health care team to achieve the best possible health outcome.

- Experience in medical social work
- Experience working with people living with HIV
- Experience with marginalized populations in a health-related field
Education
- Bachelor of Social Work (BSW)
Licenses
- Valid Class 5 driver's license
