Job Description: Reporting to the Manager or designate, the incumbent will provide services to adults who are involved in Primary Health Care programming. The Social Worker - BSW provides services as a member of a multidisciplinary team. The incumbent will work collaboratively with community groups, agencies, and other professionals within the community.
