Job Description
JOB SUMMARY

The Public Health Nurse (PHN) works within a multi-disciplinary team to provide services in health promotion, health protection, and disease treatment and prevention for individuals, families, groups and communities, under the direction of the Senior Leadership Team of Niagara Region Public Health and Emergency Services (NRPH & EMS). The PHN combines knowledge from public health, social and nursing sciences to engage in promoting health, assessment, planning, implementation and evaluation of public health interventions and services. Services are provided virtually and in person through home or office visits, community settings and presentations, agency liaison and collaboration.

The PHN is guided by departmental priorities, the Ontario Public Health Standards, the Canadian Public Health Association competencies, Community Health Nursing standards and the College of Nurses of Ontario standards.

The PHN works according to the College of Nurses of Ontario’s Practice Standards and program specific plans, policies, procedures and guidelines, while adhering to corporate and departmental policies.

QUALIFICATIONS

EDUCATION

  • Baccalaureate degree required in Nursing (BScN/BN)

  • Current Registration as a Registered Nurse with the College of Nurses of Ontario

  • Current certificate in CPR(HCP/BLS)

KNOWLEDGE/EXPERIENCE

  • Experience in a community health or public health setting is an asset

  • Experience working with client populations as defined under the specific program requirements

  • Knowledge of infection prevention and control interventions

SKILLS

  • Past accomplishments will demonstrate excellent communication skills (verbal and written), group presentation and facilitation skills, counseling skills and the ability to work effectively on a multidisciplinary team

  • Demonstrated knowledge of the social determinants of health and experience working with vulnerable populations

  • Demonstrated proficiency in competencies of community health nursing

  • Demonstrated ability to participate and contribute to shared health promotion program plans created in multidisciplinary teams

  • Skills in vaccine administration

  • Computer literacy is required

  • Fluency (written and spoken) in French and other languages is considered an asset

SPECIAL CONDITIONS

  • Work flexible hours including weekend, standby (on-call) and/or evening hours

  • Vaccinations as outlined in departmental policy

  • May be redeployed to nursing work across the Department as required

  • Must provide own transportation to be able to travel throughout the Niagara region and must possess and maintain a valid driver’s license to the class of vehicle operated
