Occupational Therapist - Slaight Dementia Project - Temporary Full-Time 2024-10759

This is an exciting temporary full-time opportunity (contract ending August 31, 2027) to work with an interprofessional primary care team in the community as a member of the Community Psychiatric Services for the Elderly (CPSE). The OT will focus on working with residents with dementia and at risk of dementia in 4 Toronto Senior Housing Corporation (TSHC) buildings. The OT will work closely with team members from Sunnybrook and partners of the North Toronto Ontario Health Team. In collaboration with other team members, the OT will:

 

  • Conduct Occupational Therapy assessments to enable seniors to participate in community living skills as safely and effectively as possible and to provide psychosocial support.
  • Practice in accordance with regulatory body (COTO) Standards of Practice.
  • Develop and implement individualized treatment plans to stabilize symptoms and function.
  • Collaborate with clients, families and caregivers to promote seniors living in their community.
  • Act as an Occupational Therapy expert resource for staff, students, and other healthcare providers.
  • Participate in team meetings, rounds, teaching, and family conferences to review client care.
  • Maintain and upgrade knowledge and skills through continuing education and professional development.
  • Seek opportunities for involvement in practice-based research and engage in knowledge translation through evidence-based practice.

 

Participate in profession-specific and inter-professional activities in support of the Sunnybrook Occupational Therapy service and the Department of Occupational Science & Occupational Therapy at the University of Toronto.

 

Qualifications & Skills

 

  • Familiarity working with seniors; knowledge and experience related to the target population is an asset.
  • Experience facilitating group sessions for seniors.
  • Strong documentation skills and well versed in computer functions, typing speed 40-60wpm.
  • Strong ability to work in an interprofessional team environment, with well-developed communication and interpersonal skills to manage conflict and optimize clients experiences.
  • Ability to manage multiple tasks, set priorities, and be flexible in an ever-changing environment.
  • Strong commitment to inter-professional collaboration, to facilitate an excellent client care experience.
  • Demonstrated commitment to person-centred care principles and best practices.
  • Ability to demonstrate reflective practice.
