Job Description:
Working as a collaborative member of the Interprofessional Primary Care Team (IPCT), the Community Ambassador functions to support the care needs of our community who do not have a primary care provider. The Community Ambassador will be a liaison between patients, families, and the healthcare
team to ensure a positive and informed care experience. This role involves providing support to patients to attend appointments when needed, manage appointment reminders, and address any concerns. The ideal candidate is empathetic, detail-oriented, and committed to fostering a welcoming and inclusive
environment. The Community Ambassador will work in collaboration with the IPCT to remove barriers to care, provide community outreach, and develop positive relationships with community members and external Ontario Health Team (OHT) partners.
The Community Ambassador will:
• Supervise and provide coaching to volunteers and staff
• Engage residents in highly impacted Scarborough communities with a focus on those who are unattached to a primary care provider, to understand their needs as it relates to health, wellbeing and access to care.
• Understand the social, economic, and cultural conditions of the immediate community and neighbourhoods and demonstrate commitment to the goals and principles of diversity, equity, and inclusion.
• Support the development and implementation of collaborative care plans
• Disseminate information and resources on health promotion focusing on the social determinates of health (food and housing insecurity, education)
• Offer health education adapted to individual audiences (culture, life history) to support health literacy.
• Assist residents to access testing, screening, and community health resources i.e.,Transportation.
• Identify and incorporate resident concerns and work closely with the IPCT and OHT partners.
• Contribute to program evaluation efforts including documentation and reporting.
• Identify, create, and deliver relevant health messaging and workshops based on resident’s
needs.
• Perform other duties as assigned that are reasonable within the scope of the job.
Requirements:
• Experience working with marginalized populations using a community development model including meaningful outreach and engagement experience.
• Strong organizational skills and a passion for helping others.
• Post-secondary education in health-related field preferred (i.e., Health Promotion, Social Service Worker)
• Understanding of community concerns, safety issues and local resources is important for effectively addressing patient circumstances.
• Knowledge of community resources and referral pathways to the appropriate services necessary to best serve the person.
• Excellent verbal communication skills
• Demonstrated training and experience facilitating meetings/groups/presentations and engaging
with adults and families.
• Demonstrated experience/confidence in working independently, with high degree of selfdirection and strong organizational skills.
• Thorough knowledge of broad determinants of health and issues affecting low income, multilingual, and racialized and LGBTQ communities.
• Excellent interpersonal, communication, decision making, problem solving, conflict
management, teamwork, and time management skills.
• Excellent work performance and attendance record
• Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a
healthy and positive work environment in accordance with Health & Safety policy and
legislation.
• Utilize protective equipment provided and identify and reports hazards.
• Engages in ethical conduct and strives to understand and appreciate the diversity of our
patient/staff population and community.

• Crisis Prevention Institute Training
• Chronic disease self-management training preferred.
• Diversity, equity, and inclusion training preferred.
• Ability to speak another language.
• Access to vehicle and valid driver’s license preferred
