Position Summary:
In accordance with established standards, professional requirements and as a member of the multidisciplinary health care team the Discharge & Flow Coordinator provides administrative and clinical leadership to all staff in a supportive, empowering manner while facilitating the effective and efficient movement of admitted patients through the hospital system. Responsible for planning, organizing, directing, coordinating, and evaluating activities related to bed utilization and patient discharge. The emphasis is on providing support to teams across acute and post acute sites in discharge planning for longer stay patients , streamlining acute care referrals and allocations of patients to Transitional Care Units (TCUs) at the University Health Network (UHN). Ensuring that patients are assigned to the most suitable TCUs efficiently and effectively. The primary site will be based out of Toronto Western Hospital. UHN has multiple locations across Toronto and you may be required to work at other locations periodically as needed.
