Vancouver Coastal Health is looking for a graduate of a Social Services Worker Program to join the team as a Community Liaison Worker in Vancouver, BC. Apply today to join our team!

As a Community Liaison Worker with Vancouver Coastal Health, you will:

  • Work as a member of the interdisciplinary team to provide a variety of advocacy/liaison services to clients within the designated program living with health issues including those who are taking HIV medications.
  • Screen clients for eligibility for services, link clients to appropriate internal and external resources and advocate for clients with various community and/or government agencies.

About Intensive Health Outreach Team (IHOT):

IHOT is for people needing additional supports with health and housing. This team will initially focus on people experiencing homelessness who are transitioning into housing (i.e., hotels, modular housing and VCH Mental Health and Substance Use supported housing) from Oppenheimer Park. On-going work will focus to other supported housing sites around Vancouver Community.

Qualifications

Education & Experience

  • Completion of Grade 12, plus completion of Welfare Aid or Social Services Certificate from a recognized college and one (1) to two (2) years’ experience working with marginalized client populations with complex health and/or psychosocial problems, or an equivalent combination of training and experience.
  • Valid BC Driver’s license and access to a personal vehicle for business-related purposes, as required.

Knowledge & Abilities

  • Knowledge of community resources and services.
  • Knowledge of related issues facing clients starting or receiving HIV anti-viral therapy.
  • Working knowledge of the social, emotional and health-related problems of the client population.
  • Knowledge of medical terminology.
  • Ability to keyboard 5,000 keystrokes per hour.
  • Demonstrated ability to work under pressure, work independently with minimal direction, problem solve, take initiative, anticipate potential problems/conflicts and take appropriate actions, organize and prioritize work, and meet deadlines.
  • Demonstrated ability to establish satisfactory work relationships with the public and other employees, to deal tactfully and effectively with co-workers, public and staff from other agencies, to maintain confidentiality and to communicate effectively both verbally and in writing.
  • Ability to make mathematical computations with reasonable speed and accuracy.
  • Exceptional customer service and communication skills.
  • Ability to work within a team environment.
  • Ability to operate related equipment.
  • Physical ability to perform the duties of the position.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Successful applicants may be required to complete a Criminal Records Review Check.

As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
