Benefits
Coverage includes 80% of basic services (maximum $1,500 per calendar year), such as recall exams, fillings, root canal; 50% of major services (maximum $1,500 per calendar year), such as dentures, crowns; and 50% of orthodontic services (maximum $2,000 per lifetime, age 6 – 18 only). Enhanced Plan – Dental Care: Employees can pay an optional premium to ‘buy up’ to Enhanced Dental Care. Coverage includes 100% of basic services; 80% of major services and 2 recall exams per calendar year. The same plan maximums apply.
Long-Term Disability: (Mandatory) Employee pays 100% of premium. Cash benefits received are tax-free. Subject to approval of medical evidence by SunLife Financial. Coverage begins after 17 weeks of continuous sickness or disability. Benefit amount – 60% of your first $5,000 of monthly earnings, plus 40% of any excess amount, to a maximum of $6,000. Benefit period – to age 65. Short-Term Disability (Self-insured): The Company self-insures a Short-Term Disability plan for permanent employees who have completed one (1) year of employment and are unable to work due to verifiable, non-occupational medical cause. Subject to approval by a leading care management company. Benefit amount – 100% of weekly earnings for each year of completed service; 66 2/3% of weekly earnings for each week of disability thereafter. Maximum benefit period – 17 weeks.
Group Life Insurance: Coverage is 1 x annual earnings to a maximum benefit of $200,000. Extended Health coverage will continue to the spouse and eligible dependents for one (1) year. Dependent Life Insurance: Coverage is $5,000 for spouse; $2,500 each dependent child. Optional Life Insurance – Employee & Spouse: Optional (At Employee’s Expense). Coverage available in units of $10,000 to a maximum (combined) benefit of $500,000 (for you and/or your spouse). Approved by Underwriter.
Group Insurance Benefits: Enrolment in the Group Insurance Benefits Plan is mandatory for permanent, full-time employees upon successful completion of a three (3) or six (6) month probationary period (as stated in offer of employment contract). Liburdi pays 100% of the premiums for your extended health coverage, dental care, vision care, group life, dependent group life, and accidental death and dismemberment. Long Term Disability is the only benefit which is not funded by the Company so that disability payments you receive will be non-taxable.
Extended Health Coverage: Coverage includes 80% (lowest cost generic drugs available); semi-private room accommodation; private duty nursing; paramedical services (including chiropractor, podiatrist, etc.); orthotics, vision care, eye exams and other medical services. Annual and per visit maximums apply. Global Medical Assistance coverage.
Group Retirement Savings Plan (RSP): A professionally managed, self directed retirement savings plan registered with Revenue Canada. Employee contributions are invested on a tax-deferred basis, until withdrawn. Contributions are voluntary and can begin immediately upon commencement of employment. The purchasing power of the group program means lower Investment Management Fees. Group Pension Plan: Mandatory participation after twenty-four (24) months of continuous permanent employment. Company will contribute between 1% to 2% of salary based on years of service. The Company will match employee contributions up to another 4% of salary after twenty-four (24) months of continuous permanent employment. Company contributions to the pension plan in respect of an employee become vested after two (2) years participation in the plan.
Year End Discretionary Bonus: At the sole discretion of the Company, individual bonuses may be awarded to employees for outstanding contributions.
Quarterly Incentive Bonus Plan: Intended to provide a fair method of sharing profits with employees. Paid out based on achievement of quarterly financial goals and contribution of individual employees. Eligibility – minimum of six (6) months permanent employment, must have received rating of three (3) or greater on most recent performance evaluation and must be actively employed at date of pay out.
Educational Assistance Program: Pre-approved educational courses, seminars or programs; 100% is reimbursed for courses which have direct application to job/career development and 50% for courses that have limited job application. Reimbursement for course fees, registration fees, books will be made subsequent to successful completion (pass). Eligibility – six (6) months permanent employment.
Service Award Program: Awards based on years of service are provided to all staff celebrating milestone anniversaries (10 & 25 years of service).
